The Silent Threat: Prompt Injection in Healthcare

As healthcare institutions increasingly integrate Large Language Models (LLM) to assist in clinical decision-making and patient communication, a new frontier of cyber risk has emerged: Prompt Injection.

What is Prompt Injection?

Prompt injection occurs when an attacker provides specially crafted input to an LLM that tricks it into ignoring its original instructions and executing unintended commands. In the context of LLMs, this can bypass safety filters and manipulate the model’s output.

The Impact on Healthcare

In the healthcare sector, the stakes of a prompt injection attack are incredibly high. The consequences can range from administrative disruptions to direct threats to patient safety:

  • Manipulation of Medical Advice: An attacker could inject prompts into a patient-facing chatbot to alter medical recommendations, potentially leading to dangerous self-treatment.
  • Unauthorized Access to PHI: If an LLM is connected to electronic health records (EHRs), a prompt injection could be used to trick the model into revealing sensitive Protected Health Information (PHI) of patients.
  • Bypassing Safety Guardrails: Medical assistants designed to follow strict safety protocols could be manipulated to ignore critical contraindications or drug-drug interaction warnings.

Conclusion

As we embrace AI in medicine, security must be a foundational component. Protecting against prompt injection is not just a technical necessity; it is a critical part of patient care and data privacy.
